Sam and Castiel’s first encounter is incredibly sweet as well as intense. Sam is in awe of this being that saved his brother from Hell, and who is a physical representation of the higher power that he has chosen to believe in. Sam has always believed in God and angels, and here one is standing before him. And the one who saved Dean to boot. Of course, Sam feels grateful towards him, even transfixed. Cas’s greeting is simple but friendly, or as friendly as an angel could be at this stage. He doesn’t treat Sam with disdain like Uriel had done and doesn’t shrink back in his presence. When Sam sticks his hand out for him to shake, Cas doesn’t immediately return the gesture. This isn’t because Castiel is hesitant to touch him. It takes him a second to recognize the greeting and that Sam is being receptive to him. There’s a slight smile on Cas’s face like ‘Oh. That’s what he’s doing,’ and that’s when he finally clasps Sam’s hand within his own. The grip is tight but not painful, signaling likewise respect and receptiveness.

While Uriel is relishing in beating up Dean, Cas has the opposite reaction when faced with Sam. Even with Cas advancing towards him, Sam has no intention of trying to defend himself, instead choosing to try and placate Cas. But rather than fight him, Cas instead knocks Sam out, temporarily. It’s a small moment but it’s an early example of Cas trying not to harm Sam, but protect him from it.
